Discussed with the provider : Ideas for Use of the Attendance Form and crossing out when the child leave so that the form always shows exactly who is present at any time.
An unannounced monitoring inspection was conducted beginning at 12:30 and ending at 1:45. There were no children in care today but the provider has three childrern enrolled, two infants and one toddler. Evidence: The forms for all three children are missing the first page of the Child's records, the Information and agreement form and the medication policy, Child #1 is missing a physical Plan of Correction: The provider will go on the DSS web site and get the last of the required paperwork, have the parent's complete
