Technical Assistance:
Discussed with provider the following: new renewal application, annual fees, fireplace inspection, MAT training in the event a child is registered with medications, the form online for reporting serious injury.

Comments:
An unannounced monitoring inspection was conducted today between 9:30am to 11:30am. There were 9 children (32 points) in direct care of 3 caregivers. The children were observed playing with a variety of toys including but not limited to musical rattle toys, soft blocks and home living. Emergency drill records were reviewed. A selection of staff/ household members and children records were reviewed. There were no medications on site. The family day home was maintained in a clean and organized manner. Previous violations were corrected. Attendance records were maintained. Areas of non-compliance are discussed in the violation notice. Standard #: 22VAC40-111-750-A
Description: Based on observation and record review, written parental permission was not obtained by the provider. Evidence: 1. Skin moisturizers were being used for children in care without the written parental permission. There were no records in the children's files to show that the provider had obtained written parental permission. 2. 5 containers of skin moisturizers were stored in the lower shelf of the diaper changing table. The containers were easily accessible to the children.

Plan of Correction: We will obtain the written permission from the parents and move the ointments to an inaccessible area.
