An unannounced monitoring inspection was conducted today. The provider and two assistants were caring for 12 children, totaling 27 points. Children's records, caregiver and household members background checks, caregiver training, children's nutrition, the physical plant, and emergency preparedness items were inspected today. The children were observed playing with toys, enjoying outdoor play, and preparing for lunch. The children were served red beans, ground turkey, zucchini, and milk for lunch today. This inspection was completed between approximately 10:30am and 11:45am. Thank you for meeting with me today. If you have any questions regarding this inspection please email me at email@example.com
Standard #: 22VAC40-111-80-A Description: Based on a review of children's records, it was determined that the provider has not documented that she viewed one child's original proof of identification within the child's first seven days of attendance. Evidence: Child care entered care on 5/7/14. There is no documentation to show that the provider has viewed the child's original proof of identification. Plan of Correction: I saw his birth certificate when he first enrolled. I will ask the parent to show it to me again and document it tomorrow.
