Technical Assistance:
Discussed with Provider that violations should be corrected in a timely manner to avoid repeat violation and being placed on provisional license.

Today an unannounced monitoring inspection was conducted with the Provider. This visit took place in the morning between 11:45am-1:15 there were 8 children present with the Provider and 2 assistants for a total of 28 points within the supervision guidelines. The children were observed having outdoor free play, lunch and nap time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, attendance record, and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-60-B
Description: Based on review of five children?s records, the provider did not maintain complete children?s records. Evidence: Child #1 & #3's record did not include documentation of two designated emergency contact person's complete address., parents work address and phone number.

Plan of Correction: Provider stated that she will get the information from the parent.

Standard #: 22VAC40-111-530-F
Description: Based on observation of the infants during nap time, stuffed toy was fond under a sleeping infant. Evidence: Upon arrival, an infant was observed in a crib sleeping with a stuffed animal/toy under the infant.

Plan of Correction: Provider immediately removed during this inspection and will make sure to inform all staff that items are not allowed in cribs.

Standard #: 22VAC40-111-690-D
Description: Based on walk thru of physical plant, the diapering surface was ripped/torn and no longer non-absorbent. Evidence: The diaper changing pad used for diapering was longer non absorbent.

Plan of Correction: Provider replaced immediately. and discarded the old one.
