Technical Assistance:
The provider asked about the form for updating the DSS Search of the Central Registry. The inspector showed the provider the form on the web site (www.dss.virginia.gov).

Comments:
An unannounced monitoring inspection was completed on 5/14/18. There were five children present with the provider supervising. The point total for the children in care was 10 at arrival of the inspector and 12 after an additional child arrived. An assistant arrived at approximately 8:45 am. Five children's records, the provider's record, and one assistant's record was reviewed. The inspector arrived at the provider's home at 7:30 am and departed at 10:00 am. The children were observed at arrival, during breakfast, during free play time, and during departures for school. There was discussion with the provider about the following: updating background checks and the new Sworn Statement form, the information on the Supplemental Information form and not posting this part of the inspection, about the physical required within 30 days for children (child #1 had a notice from the physician of a scheduled appointment for a physical examination).

Violations:
Standard #: 22VAC40-111-730-D
Description: Based on observation and interview, the provider failed to ensure that medication was kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. The provider was asked by the inspector if there were any household medications on this floor of the house. The provider showed the inspector a prescription medication that was stored in a prescription bottle unlocked in her purse that was accessible to the children. The provider's purse was located in a chair that was approximately three feet tall in the child care area. The purse was not locked in a way that would prevent access by children.

Plan of Correction: This was corrected during the inspection. The provider locked the medication in a cabinet in the kitchen.

Standard #: 22VAC40-111-920
Description: Based on observation and interview, the provider failed to ensure that the menu for the current one-week period was posted or available. Evidence: 1. There was no menu observed to be posted. 2. The provider was asked about the menu. The provider stated that the menu had not been completed or posted.

Plan of Correction: The provider will post the menu for the week by the end of the day.

Standard #: 22VAC40-111-90-C
Description: Based on record review, the provider failed to ensure that documentation of additional immunizations, as required once every six months for children under the age of two years, was obtained for a child. Evidence: 1. The record of child #3 last documentation of immunization was 9/23/16. The child was four months old at this time. No other documentation of additional immunizations was documented in the child's record.

Plan of Correction: The provider talked with the parent during the inspection about getting the most current immunization record. The parent stated that the child had an appointment in June for an update. The current immunization record will be obtained from the physician at that appointment.
