Comments:
An unannounced renewal inspection was conducted beginning at 8:15 and ending at 9:40. There were 6 children in care including a child who lives in the home. The provider and a family member were providing care. Children were observed eating breakfast , watching an educational video and singing songs. Reviewed files - see Violation notice, Family Files are complete Emergency drills are up to date. Provider does not give medication. Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation, there are two children in care who drink from bottles. The bottles were not labeled with the child's name and the date.

Plan of Correction: All bottles will be labelled with the child's name and the date.

Standard #: 22VAC40-111-60-B
Description: Based on a review of the records for 5 children it was determined that not all files were complete. The File for Child #1 had only one emergency contact; the file for Child #2 had no insurance policy number; the file for child 33 did not have a physical.

Plan of Correction: The provider will contact the parents for the information.
