Standard #: 22VAC40-111-830-A
Description: Based on record review and interview, the provider did not document emergency evacuation drills monthly. Evidence: The last documented fire drill was dated 12/2017. The provider states that she has done additional drills but doesn't know where the log is.

Plan of Correction: I have done it through April, but I don't know where I put it.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information. Evidence: Child #1, Child #3 and Child #4's files did not contain the children's first date of attendance. Child #1, Child #2 and Child #3's files did not contain documentation of a review of emergency contact information within the last year. Child #3 file did not contain a street address for their second emergency contact.

Plan of Correction: I will call the parents and let them know.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not obtain documentation of adequate immunizations after a child's sixth birthday. Evidence: Child #3 is seven years old and their most recent documented immunizations are dated 9/28/2012.

Plan of Correction: I'm going to call them to get that for me this week.

Standard #: 22VAC40-111-240-A
Description: Based on observations, areas of the outside play area are not kept in a safe condition. Evidence: The backyard is covered with a green turf material. Several areas of the turf are rippled and worn causing tripping hazards.

Plan of Correction: I have furniture that will pull it it straight and nail it down so that the wind won't blow it.

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous substances were not stored in an area inaccessible to children. Evidence: At the start of the inspection, there is a large closet that is open in the entrance to the family day home. A shelf in the closet contains several cleaning agents. The cleaning materials are accessible to the child in care.

Plan of Correction: I will keep that door closed.

Standard #: 22VAC40-111-680-B
Description: Based on observation, children's hands were not washed with running water. Evidence: Prior to eating lunch, one child was observed having their hands washed in a bucket with soap and water.

Plan of Correction: I thought because they are small that it is okay. I will send the small ones to the sink.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the provider did not have the most recent inspection summary and violation notice posted. Evidence: When asked about the findings from the most recent inspection, the provider was able to locate the last inspection summary and violation notice that was stored in a binder.

Plan of Correction: Okay. I can do that.
