An unannounced monitoring inspection was completed on 05/16/2018. There were 8 children, ages 2 years-10 years, totaling 12 points present during today's inspection. There was one caregiver providing direct care and supervision during the inspection. There inspector reviewed 5 children's records and one provider record during today's inspection. There is no medication being administered at this time. The children were observed in the following activities: nap time, afternoon snack and free choice activities. The school-aged children were observed arriving to the afterschool care via public school transportation. The emergency preparedness plans and emergency supplies were reviewed. Proper hand washing and table sanitization were also observed. The inspector discussed the new VDSS model forms available to providers and annual training with the provider. An exit conference was held at the conclusion of today's monitoring inspection. The inspector arrived at 12:45 p.m. and departed at 3:30 p.m. Thank you for your time today. Description: Based on observations and discussion with the provider, the provider failed to ensure that the phone number of the person designated as the emergency backup care, as required in 22VAC40-111-800-A-3, was posted in a visible area close to the telephone. Evidence: The name and phone number of the emergency backup care provider was not posted in a visible area close to the telephone. The information was posted on a bulletin board and had been covered by other papers. The bulletin board where the information was posted was not located near the telephone.

Plan of Correction: The provider will move the information to an area by the telephone.
