Discussed napping, background checks, and caregiver requirements.
An unannounced monitoring inspection was conducted today from 12:15pm-3:45pm. There were 8 children (27 points) supervised by 3 direct care staff. The home, 3 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed napping, eating snack, playing with cars and various interactive toys, and listening to music.. Diapering procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Standard #: 22VAC40-111-120-B Description: Based on record review, the assistants' records did not contain all required documentation. Evidence: During the time of inspection, Staff B (date of hire: 5/21/2018) and Staff C's (date of hire: 5/21/2018) records did not contain documentation of emergency contact information and completed orientation. Plan of Correction: I will complete it tomorrow.
Standard #: 22VAC40-111-60-B Description: Based on record review, 4 of 5 children's records were incomplete. Evidence: 1. During the time of inspection, Child #1's (start date: 11/27/2017) record did not contain documentation providing proof of the child's identity. 2. During the time of inspection, Child #2's (start date: 9/25/2017) record did not contain documentation of addresses for both emergency contacts, and complete medical insurance information. 3. During the time of inspection, Child #4's (start date: 6/29/16) record did not contain documentation of acknowledgement of the established emergency preparedness plan. 4. During the time of inspection, Child #5's (start date: 8/10/16) record did not contain work phone numbers for both parents. Plan of Correction: I will ask the parents tonight.
Standard #: 22VAC40-111-140-B-3 Description: Based on record review and provider interview, there was no documentation of completed CPR as required. Evidence: During the time of inspection, there was no documentation of Staff A's completed CPR. Plan of Correction: Provider's daughter has documentation of provider's completed CPR and will bring it.
Standard #: 22VAC40-111-170-A Description: Based on record review, the provider did not obtain a report of a current TB screening from each caregiver at the time of hire. Evidence: During the time of inspection, there was no documentation of a current TB screening for Staff C (hire date: 5/21/2018). Plan of Correction: Caregiver has it at home and will send a copy.
Standard #: 63.2-1720.1-B-2 Description: Based on record review, there was no documentation of completed fingerprinting prior to their first day of employment, as required. Evidence: During the time of inspection, there was no documentation of completed fingerprint background check for Staff B (date of hire: 5/21/2018) and Staff C (date of hire: 5/21/2018). Plan of Correction: I will send them this week.
