Comments:
An unannounced monitoring inspection was conducted today from 12:05pm-2:30pm. There were 7 children (24 points) supervised by 3 direct care staff. The home, 3 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed singing with musical instruments, playing outside on the playground, eating lunch, and reading stories. Diapering procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-960-E
Description: Based on observation, 2 infant bottles were not properly labeled with all required information. Evidence: Child #2 and #4 had bottles that did not contain the date.

Standard #: 22VAC40-111-540-A
Description: Based on observation, an infant high chair was occupied by a child without the safety strap being used and securely fastened. Evidence: During the time of inspection, Staff B placed Child #4 in a high chair and fed her without fastening the safety strap.

Standard #: 22VAC40-111-680-A
Description: Based on observation, caregivers did not wash their hands with soap and warm running water as required. Evidence: 1. Staff B used a facial tissue to clean a child's nose, and then began feeding the child. The staff member did not wash their hands prior to feeding the child. 2. Staff A did not wash her hands before beginning to feed a child.

Description: Based on observation, caregivers did not ensure that children's hands were washed with liquid soap and warm running water as required. Evidence: During the time of inspection, all children were served lunch and began eating without washing their hands.
