An unannounced monitoring inspection was conducted on May 21, 2018 during the hours of approximately 10:30 a.m. and 1:40 p.m. Twelve (12) children were enrolled and ten children were in the care of the provider and three assistants during the on site inspection. Two resident children were also present in the home (total of 35 points). Five children's records, four caregivers' records and one household member's record were reviewed and/or updated. No medications were on site/administered on this date; one child's medication authorization form was reviewed. Facility documents were reviewed, safety equipment was observed (first aid kit, flashlight, radio and fire extinguisher) and posted information was viewed (license, licensing inspection documentation, daily schedule, weekly menu, emergency telephone numbers, and emergency evacuation diagram/plan). Program observation was completed. Activities observed on this date included: arrival of one child; indoor play; outdoor play; diapering, restroom and handwashing procedures; naptime; and infant care/feeding. Adult/child interactions, behavior guidance and daily routines/transitions were also observed. The children followed the daily routine and transitions well. Refer to the accompanying violation notice for the exceptions to compliance (background clearances and tuberculosis screenings). Areas of discussion included: background clearances, reviewing children's records to ensure complete information is obtained and the location of a new small climber/slide. Standard #: 22VAC40-111-170-B Description: Based on review of four caregivers' records and one household member's record and discussion with the provider on May 21, 2018, the program failed to obtain from one caregiver, at the time of hire, a tuberculosis (TB) screening/exam report that was completed within the last 30 days and signed by a physician, physicians' designee, or an official of the local health department. Evidence: 1. Review of caregiver #4's record, on this date, determined that a TB exam report/screening was completed five months prior to the date of hire (date of employment - 1/22/2018). 2. Standard #: 22VAC40-111-180-A Description: Based on review of one household member's record and discussion with the provider on May 21, 2018, the program failed to obtain a current report of tuberculosis (TB) screening every two years from the date of the first screening/exam. Evidence: 1. Review of the household member's record, on this date, determined that an updated TB screening/exam report was not on file. 2. Standard #: 63.2-1720.1-B-2 Description: Based on review of four caregivers' records and one household member's record and discussion with the provider on May 21, 2018, the program failed to obtain fingerprinting results for two caregivers prior to employment . Evidence: 1. Review of caregiver #3 and caregiver #4's records, on this date, determined that fingerprinting results were not on file. Caregiver #3's date of hire was 2/19/2018 and caregiver #4's date of hire was 1/22/2018. Criminal history record clearances were on file for the caregivers. 2. Standard #: 63.2-1720.1-B-3 Description: Based on review of four caregivers' records and one household member's record and discussion with the provider on May 21, 2018, the program failed to obtain copies of the results of any child abuse and neglect registry or equivalent registry maintained by any other state in which one caregiver resided in the preceding five years. Evidence: 1. Review of caregiver #4's record, on this date, determined that the caregiver resided in two states (other than Virginia) within the preceding five years. Copies of the results of searches of any child abuse/neglect registries maintained by those states were not requested/obtained within 30 days of employment (hire date - 1/22/2018). 2. The provider confirmed that searches of any child abuse/neglect registries maintained by the states in which caregiver #4 had resided had not been submitted or obtained.
