Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and a review of changes to background checks with regards to expiration dates, new fingerprint checks to be done through Fieldprint of Virginia & Office of Background Investigations (OBI), new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. The roll out for the new fingerprint checks occurred on January 22, 2018. All finger print checks for existing employees household members must be started for each according to the established time table and then completed by September 30, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file. If you did not receive your Facility ID number, your Field Print code numbers or you have not establish a point of contact, then please contact the Office of Background Investigations (OBI) at 1 (804) 726-7884.

The following inspection took place between the hours of 11 am through 1:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 5 children (16 points) with 2 caregivers. A total of 8 children?s files, 2 staff files, and 1 household member?s file was reviewed. The children were engaged in the following: toy play, snack time, story time, music, song, infant feeding, infant sleeping, lunch time, diaper changing, hand washing, bathroom breaks, and other organized activities. Standard #: 22VAC40-111-800-B
Description: Based on observation, the most recent date of a documented review of the Family Day Home's (FDH) Emergency Preparedness & Response Plan (EP&RP) was older than one year. Evidence - 1. On the date of inspection (06/01/2018), the most recent EP&RP review was dated - 07/27/2016. 2. The EP&RP was observed posted on the FDH bulletin board which was located on the wall near the refrigerator.

Plan of Correction: The EP&RP shall be reviewed and the date shall be recorded.

Standard #: 22VAC40-111-30-A
Description: Based on observation, the Provider did not ensure compliance with the terms of the current license issued by the department. Evidence - 1. On the date of inspection (06/01/2018), the current license issued by the department was observed posted on site at the Family Day Home (FDH) with an age range set as - 3 months through 3 years of age. 2. On the date of inspection, Child #8 was in attendance. Based on the information on the file, Child #8 is 4 years of age.

Plan of Correction: Child #8 will either no longer attend or a modification shall be requested to expand the age range of the current license.

Standard #: 22VAC40-111-180-A
Description: Based on review, 1 of 3 staff/household members files reviewed indicated that the Provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis (TB) Screening form (in accordance with the requirements in 22 VAC 40-111-170), every two years from the date of the most recent documentation. Evidence - 1. On the date of inspection (06/01/2018), Household Member #1 most recent TB documentation was dated - 04/12/2016. 2. This review was confirmed on the date of inspection by the Provider.

Plan of Correction: Updated TB documentation shall be obtained by Household Member #1 and placed on file.
