An unannounced monitoring inspection was conducted today from 9:25am-11:30am. There were 11 children (37 points) supervised by 3 direct care staff. The home, 3 staff records, 6 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed napping, playing with puzzles, dancing, playing with push toys, and engaging in free play. Diapering procedures and bottle feedings were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Standard #: 22VAC40-111-60-B
Description: Based on record review, 1 of 6 children's records did not contain all of the required information. Evidence: Child #1's (start date: 3/26/2018) record did not contain documentation of parent work telephone numbers.

Plan of Correction: I will ask the parents this afternoon to put the phone numbers on the papers.

Standard #: 22VAC40-111-180-A
Description: Based on record review, for 1 of 3 records, the provider did not obtain a current report of tuberculosis screening form every two years from the date of the first screening. Evidence: Staff B's (date of hire: 9/2001) record contained documentation of the most recent TB screening conducted on 9/21/2018, which would have expired 9/21/2017.

Plan of Correction: I will tell the caregiver to get her TB test done next week.

Standard #: 22VAC40-111-350-E
Description: Based on observation, hot water taps available to children were not maintained within a range of 105 to 120 degrees Fahrenheit. Evidence: The hot water tap in the children's restroom was measured at 135 degrees Fahrenheit.

Plan of Correction: I will have the temperature adjusted today.

Standard #: 22VAC40-111-680-B
Description: Based on observation, caregivers did not ensure that children's hands were washed with liquid soap and warm running water after toileting or diapering. Evidence: Staff B was observed changing Child #6's diaper, and did not wash the child's hands with liquid soap and warm running water after completing the diaper change.

Plan of Correction: We will start handwashing after diapering today.
