The licensing inspector conducted an unannounced monitoring inspection on Wednesday, June 6, 2018 from approximately 12:00 pm until 3:00 pm to determine the provider's compliance with licensing standards at the family day home. During this inspection, there were 11 children present totaling 20 point being supervised by two caregivers. The licensing inspector observed the children in care participating in a variety of activities to include working on a math worksheet, engaging in play with peers at the outdoor play area, transitioning indoors for lunch, completing handwashing, eating lunch, and transitioning into napping. The children's lunch today consisted of hot dogs, baked beans, pineapples, whole wheat bread or bun, and milk. The first aid kit and emergency supplies, posted emergency contacts, fire and shelter-in-place drills were reviewed by the inspector. During this inspection, there were five children's records, one provider's record, and two assistant caregivers records reviewed by the inspector. Standard #: 22VAC40-111-920
Description: Based on observations and interview with the provider, the provider did not ensure the menu for the current one-week period was posted at the family day home. Evidence: During the inspection conducted on June 6, 2018, the menu for the current one week period was not posted in the family day home. Interview with provider confirmed that the menu had not been posted for this period.

Plan of Correction: This was corrected by the provider while on site.

Standard #: 22VAC40-111-100-A
Description: Based on review of children's record and interview with the provider, the provider did not ensure to obtain documentation of a physical examination for one of four children's records within 30 days after the first day of attendance. Evidence: During the inspection conducted on June 6, 2018, a review of Child#5's record was completed and there was not documentation of a physical examination found in the record. Interview with the provider confirmed that a physical examination for the child was not available.

Plan of Correction: The provider will have parent go to the physician office to obtain a copy by the end of the week, Friday, June 8, 2018.

Standard #: 22VAC40-111-240-A
Description: Based on observation of the playground area, the provider did not ensure all outdoor equipment was maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on June 6, 2018, the multi color 7 in 1 slide equipment on the play area was observed with multiple cracks with sharp edges on the pink and blue sections of the equipment.

Plan of Correction: The children will not be able to play on the equipment. The provider will remove the 7 in 1 equipment from the play area by Friday, June 8, 2018.
