Technical Assistance:
Staff under the age of 18 must be under the sight and sound supervision of an adult staff member. VA Preservice training can be counted as annual training one time. Please notify your inspector of termination/suspension of services for more than 24 hours as a result of an emergency. Please complete http://www.dss.virginia.gov/facility/injury_report_subsidy.cgi to report any serious injury to the Division. Serious injuries are defined as an injury sustained while in day care that resulted in the child getting outside medical treatment. Please label all substitute containers containing hazardous substances with the contents (for example, bleach and water in a spray bottle).

Comments:
An unannounced subsidy health and safety inspection (SHSI) was conducted with the provider. A licensing inspection was also conducted on this date (see separate report for details). There is currently 1 assistant working at the FDH. Both caregivers have documentation of the mandatory VA Pre-service Training and both are currently certified in first aid and CPR . There are no children with diagnosed food allergies currently enrolled ? please review the applicable standards regarding food allergies. Evacuation drills have been conducted monthly, shelter in place drills conducted twice a year, and a lockdown drill was conducted 11/27/2017. Lockdown drills are to isolate children from a security threat; shelter in place drills are to protect children from a natural or manmade threat.
