Description: Based on records reviewed and interview conducted, it was determined that the maximum licensed capacity of 12 children was exceeded on 8 days between 4/16/18-5/21/18. [Photos #1, #2, #3, #4, #5, and #6] Evidence: 1. On 4/17/18, 14 children were in care, ranging in age from 7 months old - 5 years old. 2. On 4/18/18, 13 children were in care, ranging in age from 7 months old - 5 years old. 3. On 4/19/18, 13 children were in care, ranging in age from 7 months old - 5 years old. 4. On 4/25/18, 13 children were in care, ranging in age from 7 months old - 4 years old. 5. On 5/2/18, 13 children were in care, ranging in age from 7 months old - 5 years old. 6. On 5/9/18, 13 children were in care, ranging in age from 7 months old - 5 years old. 7. On 5/10/18, 13 children were in care, ranging in age from 7 months old - 5 years old. 8. On 5/15/18, 14 children were in care, ranging in age from 7 months old - 5 years old. 9. The provider acknowledged that on some days there have been more than 12 children in care.

Plan of Correction: Part-time parents were notified that during the coming preschool year, their child will only be accepted on the days listed in the contract and that they are responsible for transportation on the other days. done [sic]

Standard #: 22VAC40-111-570-A
Description: Based on records reviewed and observations made, it was determined that ratios were not maintained for approximately 41 minutes today, and the maximum allowance of 16 points per caregiver was exceeded. Evidence: 1. At approximately 2:19pm, the inspector observed the provider leaving the home and driving away. 2. At approximately 2:43pm, the inspector entered the home and determined that Assistant #1 was working alone and supervising 12 children ranging in ages 6 months - 5 years old, and totaling 27 points. 3. At approximately 3:00pm, the provider returned to the home, after Assistant #1 called her and speaking with the inspector on the phone. 4. At approximately 3:01pm, Assistant #2 returned to the home from her break.

Plan of Correction: Once I was notified that only childcare centers, not family day homes, were allowed to be over ratio during naptime we adjusted our breaks to be in compliance done [sic]
