A Subsidy Health and Safety inspection was conducted on June 13, 2018 to review supplemental health and safety requirements. There were 8 children in care on the date of inspection. Three children were in care during the day program and five children were in care during the evening hours. The evening care began at approximately 4:30PM. A sample of records was reviewed. Documentation of the practiced emergency drills was reviewed. The LI gave information to the Provider related to the subsidy model forms that are located on the DSS web site. A discussion was held with the Provider regarding caregivers' records; an active file using the model form as a check list and all current documents will assist in locating information. An inactive file to retain all other documents can be established to organize and maintain other information for the required retention period. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health an safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. The inspection was conducted from 4:00PM to 6:15PM. Violations were found during the supplemental subsidy health and safety inspection and are on the violation notice.

Description: Based on interview, the Provider verified that written lock down procedures had not been written and included in the Family Day Home's (FDH) emergency preparedness plan. Evidence: The Provider's written lock-down procedures were not available for review upon request on the inspection date.

Plan of Correction: The Provider will review the subsidy requirements and will write lock-down procedures.
