22VAC40-191 Background Checks for Child Welfare Agencies

An unannounced renewal inspection was conducted with the provider and one assistant beginning at 12:00 PM and ending at 2:15 PM. There were twelve children in care during the inspection. The children were ages 8 weeks, 6 months, 2, 2, 2, 2, 3, 3, 4, 5, 5, and 6 years old for a total of twenty-five points. The children were observed resting on cots during nap time. The children were also observed being read a story by a caregiver. An infant was observed being bottle feed and sleeping in a crib. Six children's records were reviewed. The provider's record was reviewed and in compliance. Two household members' and one staff member's records were reviewed. Two pet records were reviewed and in compliance. The most recent emergency escape drill was 5/9/18. The most recent shelter-in-place drill was 5/29/18. The first aid kit, flashlight, and weather-band radio were observed and in compliance. There were no medications on site to review as the provider does not administer medication. The provider and assistant have current First Aid and CPR certification. Standard #: 22VAC40-111-100-A
Description: Based on review of documentation, the provider did not obtain documentation of physicals for all children in care. Evidence: Child A (enrolled 3/1/18) did not have documentation of a physical examination in the file.

Plan of Correction: The provider stated that she would request the missing documentation from the parents. Once received the documentation will be placed in the child's record.

Standard #: 22VAC40-111-90-A
Description: Based on a review of documentation and interview with the provider, the provider failed to obtain documentation of immunizations for all children prior to their first day of attendance. Evidence: Child A (enrolled 3/1/18) and Child B (enrolled 5/29/18) did not have documentation of immunizations in their records.

Plan of Correction: The provider stated that she would request the missing documentation from the parents. Once received the documentation will be placed in the children's record.

Standard #: 22VAC40-191-40-D-1-b
Description: Based on a review of documentation, it was determined that not all staff had documentation of a central registry history records check. Evidence: Staff A's record (start date 4/1/18) did not contain documentation of a completed central registry history records check.

Plan of Correction: The central registry check was returned to the provider and resent to Social Services. The provider contacted Department of Social Services (DSS) during the inspection to follow up on the central registry check. The provider will continue to follow up with DSS regarding the central registry check.
