Comments:
A Subsidy Health and Safety inspection was conducted on 06/21/2018 to review supplemental health and safety requirements. Two children were in the care of the provider for a total of one point. Children were observed coloring, doing art activities, handwashing and during lunch time. A licensing renewal inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDHOM-000-005-B
Description: Based on observation and interview, the provider failed to have a written record of daily attendance with the arrival and departure time of children. Evidence: 1. Based on review of attendance records the last documented daily attendance with time of arrival and departure was in February 2018. 2. Provider verified that she had not done a written attendance records with time and date since February 2018.

Plan of Correction: The provider will start using a written attendance plan and keep on file for five years.

Standard #: VENDHOM-000-029-B-3
Description: Based on review of the emergency preparedness plan, the required written reunification component was not listed. Evidence: 1. The emergency preparedness plan did not include a reunification plan after an evacuation. 2. The provider verified that there was not a written reunification plan after an evacuation.

Plan of Correction: I will review and update the emergency preparedness plan and include the written reunification plan.

Standard #: VENDHOM-000-029-B-4
Description: Based on review of the emergency preparedness plan, the required written reunification component was not listed. Evidence: 1. The shelter-in-place procedure did not include a written reunification plan. 2. The provider verified that in the shelter-in-place procedure there was not a written reunification plan.

Plan of Correction: The provider will review and update the shelter-in-place procedures and include the written reunification plan.

Description: Based on review of the emergency preparedness plan, the required written reunification component was not listed. Evidence: 1. The lockdown procedure did not include a written reunification plan. 2. The provider verified that in the lockdown procedure there was not a written reunification plan.

Plan of Correction: The provider will review and update the lockdown procedure and add the reunification piece.

Standard #: VENDHOM-000-030-A-3
Description: Based on review of evacuation drills, the provider failed to practice a lockdown drill annually. Evidence: 1. There was no documentation of a lockdown drill completed in 2017. 2. The provider verified that there was not a lockdown drill completed in 2017.

Plan of Correction: The provider will complete a lockdown drill with children for this year.
