Technical Assistance:
Discussed with provider timely submission of the renewal application, discusses maintaining the emergency supplies.

An unannounced non-mandated monitoring inspection was conducted today between 10:15am to 11:30am. The purpose of this inspection was to assess previous violations. There were 5 children (14 points) present with two caregivers. The children were observed eating morning snack which included crackers, bananas, juice, and water. The children were also observed playing with toys and washing hands after snack. Staff/children records were reviewed in addition to emergency drill records. Description: Based on record review, provider did not obtain docuemntation of addiotional immunizations for children under the age of two. Evidence: 1. The most recent immunization records obtained for Child # 1 were dated 1/4/2017. 2. Additional immunization records were not obtained for Child # 1 every 6 months. Child # 1 is under the age of two.

Plan of Correction: I will ask the parents to bring in the additional immunization records.
