Comments:
A Subsidy Health and Safety inspection was conducted on 07/02/2018 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today from approximately 9:30am-11:40am. There were 12 children in care one of which belonged to the provider ( 30 points) directly supervised by 2 caregivers. A sample of 5 children?s records, 2 caregiver records and 3 household member records were reviewed. The children were observed during bathroom procedures, eating snack, coloring, building, engaged with caregivers and playing. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: VENDHOM-000-005-B
Description: Based on records review, a written record of daily attendance that documents arrival and departure of each child in care was not maintained. Evidence: The departure of each child was not recorded on the record of daily attendance.

Plan of Correction: I will mark out on the attendance as the children leave.

Standard #: VENDHOM-000-012-B
Description: Based on record review and interview with provider, all caregivers have not completed the Virginia Pre-Service training for Child Care Staff. Evidence: The provider stated that the training has been completed. There was no record of completion available to prove the completion of the training.

Plan of Correction: I will send you a copy of the certificates once I find them.
