Comments:
An unannounced monitoring inspection was conducted this date. A total of 8 children (24pts) with Provider and Assistant present. When the inspector arrived, the older children were completing a writing activity which involved writing number 1 to 13 on lined paper with pencils and tracing the letter "Q". Younger children were busy free playing on the alphabet carpet with legos, barbies and stuffed animals. Some children observed in exercise saucers and rotated to different areas for increased stimulation. Observation of the physical plant was made, hand washing and diapering procedures and record review. A total of 4 out of 8 children's records were reviewed and all staff and household records were also reviewed. No medications on site at this time. The provider is not MAT trained which is consistent with the medication policy for the home. No food allergies at this time. Ample equipment, toys and books maintained for the children. Thank you for your cooperation today. Please email me at: lisa.hudson@dss.virginia.gov
