54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced monitoring inspection was conducted this date. A total of 8 children (24pts) with Provider and Assistant present. When the inspector arrived, the older children were completing a writing activity which involved writing number 1 to 13 on lined paper with pencils and tracing the letter "Q". Younger children were busy free playing on the alphabet carpet with legos, barbies and stuffed animals. Some children observed in exercise saucers and rotated to different areas for increased stimulation. Observation of the physical plant was made, hand washing and diapering procedures and record review. A total of 4 out of 8 children's records were reviewed and all staff and household records were also reviewed. No medications on site at this time. The provider is not MAT trained which is consistent with the medication policy for the home. No food allergies at this time. Ample equipment, toys and books maintained for the children. Standard #: 22VAC40-111-960-E
Description: Based on observation of the physical plant, bottles were not labeled with each child's name and date. Evidence: Baby bottles for three different children were not labeled with each child's name and date.

Plan of Correction: I will label them each day when the parents bring the bottles in the morning.

Standard #: 22VAC40-111-100-A
Description: Based on review of children's records, physical exams were not obtained within 30 days of attendance. Evidence: Child #1 began care on 2/26/18, the physical exam found was not completed and not signed and dated by the physician. Child #2 began care on 8/7/17, a physical exam was not obtained to date. Child #3 began care on 1/8/18, a physical exam was not obtained to date.

Plan of Correction: I will request a physical exam for all three of these children from the parents today.
