The licensing inspector conducted an unannounced monitoring inspection from 9:15am - 11:45am. There were four children in care (two residential) for a total of six points with one caregiver. The children were observed watching cartoons, playing with the available toys, playing with play dough and coloring. Two adult's records and three children's records were reviewed. Violations from the previous inspection were also reviewed and all were corrected. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-111-90-C
Description: Based on record reviews, the family day home failed to obtain documentation of updates to child's immunizations. Evidence: 1. The inspector reviewed three children's records. One of three records reviewed (child record #1) did not contain updated information for immunizations every six months for child under two.

Plan of Correction: The provider will ask for updated immunization information for the child's record at pick up time today.

Standard #: 22VAC40-111-180-A
Description: Based on record reviews, the family day home failed to obtain repeat tuberculosis screenings. Evidence: 1. The inspector reviewed two adult's records. Both records contained tuberculosis screenings that were over two years old. Adult record #1's screening was dated 2/3/16 and adult record #2's screening was dated 3/28/16.

Plan of Correction: The provider said both adults will obtain repeat tuberculosis screenings by the end of the month.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the family day home failed to maintain the outside play area in a safe condition. Evidence: 1. The inspector observed the outside play area. There was black plastic protruding through the mulch creating tripping hazards.

Plan of Correction: The provider will cut the plastic today. The provider will apply additional bark mulch to these areas of the playground.
