Comments:
An unannounced renewal inspection was completed on 07/17/2018. There were 8 children, ages 2 years-12 years, totaling 11 points in the care of one provider during today's inspection. The inspector reviewed 5 children's records, one provider record, and one substitute provider record during today's inspection. The children were observed in the following activities: napping, free choice play, afternoon snack and reading time. The inspector reviewed the parent policies, the updated emergency preparedness plans and emergency supplies. The inspector discussed the license renewal process, volunteers, the areas used by the children and annual training opportunities with the provider. An exit conference was held at the conclusion of today's inspection. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider failed to ensure that each child's record contains all of the components required by the standards. Evidence: 1. The record for Child #2 did not contain the name and policy number of the child's medical insurance; the record did not contain the insurance policy number. 2. The record for Child #3 did not contain the following information: the name of the previous child care provider and the name and policy number of the child's medical insurance; the record did not contain the insurance policy number.

Plan of Correction: The provider will get the information from the parent.

Standard #: 22VAC40-111-210-A
Description: Based on record review and discussion with the provider, the provider failed to ensure that all caregivers shall obtain 16 clock hours of training annually in areas relevant to their job responsibilities. Evidence: The record for Substitute Provider #1 did not contain documentation of 16 clock hours of annual training; there were 6 hours of annual training documented in the record.

Plan of Correction: The provider will have the substitute provider participate in online training.

Standard #: 22VAC40-111-510-A
Description: Based on observations, the provider failed to ensure that each child shall be provided with an individual crib, cot, rest mat or bed for resting or napping. Evidence: The inspector arrived for the on-site renewal inspection during the designated nap time. Upon arrival the inspector observed a three year old child sleeping on the couch and a two year old child sleeping on the loveseat in the living area of the home; the other 4 napping children were observed sleeping on rest mats.

Plan of Correction: Starting tomorrow all of the children will sleep on the rest mats.

Standard #: 22VAC40-111-690-L
Description: Based on observations, the provided failed to ensure that toilet chairs, when used, shall be emptied promptly, cleaned and sanitized after each use. Evidence: The inspector observed urine in the toilet chair during the inspection.

Plan of Correction: The provider will be sure to empty, clean and sanitize the toilet chairs after each use. The toilet seat was emptied during the inspection.
