54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced monitoring inspection was conducted with the provider beginning at 10:53 AM and ending at 1:00 PM. There were four children in care during the inspection. The children's ages were 4 months, 6 months, 10 months, and 20 months old for a total of 15 points. The children were observed being playing with soft toys and manipulatives. Two children were observed being fed in the high chair. One child was observed being bottle fed. Four children's records were reviewed. Three household members' records were reviewed. The emergency evacuation drill logs were reviewed. There were no medications on site as the provider is not Medication Administration Training (MAT) certified which is required to administer medication. The first aid kit, flashlight, and weather band radio were observed an in compliance. Standard #: 22VAC40-111-100-B
Description: Based on a review of documentation, the provider did not obtain documentation of a physical examination for all children prior to attendance. Evidence: Child A did not have documentation of a physical examination in their record.

Plan of Correction: The provider stated that she had provided the physical forms to the parent and requested that the completed forms be returned.

Standard #: 22VAC40-111-60-A
Description: Based on a review of documentation, the provider did not maintain a complete record for all children in care. Evidence: The record for Child B (start date 1/12/17) did not have a second emergency contact. The record for Child C (start date 1/29/18) and Child D (start date 6/20/18) did not have telephone numbers for the parents' place of employment.

Plan of Correction: The provider stated that she would obtain the missing information from the parents and update the children's records.

Standard #: 22VAC40-111-180-A
Description: Based on a review of documentation, the provider did not obtain documentation of a current Report of Tuberculosis Screening form every two years from the date of the first screening. Evidence: Household member A and B's most recent Tuberculosis Screening documentation was dated July 5, 2016.

Plan of Correction: The provider stated that she would ensure that both household members have an updated tuberculosis screening conducted as soon as possible.

Standard #: 22VAC40-111-240-A
Description: Based on observation, not all of the furnishings inside the family day home were maintained in a clean condition. Evidence: There was dried food residue on one of the three high chairs.

Plan of Correction: The provider stated that she did not notice that there was food residue on chair part of the high chair and would clean the entire chair.
