54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted today from approximately 12:50pm-2:50pm. There were 8 children (20 points) in direct care of 2 caregivers. A sample of 5 children?s records, 2 caregivers records and 1 household member's record was reviewed. The children were observed dancing, singing, eating and napping. The family day home did not have required documents properly displayed because provider was redecorating as a result of a flood that occurred two months ago in the main area of care. The area is appropriate for care and age appropriate toys games and books were present. Zero new caregivers have been hired since the last inspection. Standard #: 22VAC40-111-830-C Description: Based on record review and interview with provider, documentation of emergency drills were not recorded. Evidence: The evacuation drills that were conducted in May and June were not properly recorded. Weather conditions, amount of children that participated, and times that the drills took place was missing from the record. Plan of Correction: I will update the record today.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records were incomplete. Evidence: 4 of 5 children's records were missing required information. 1) Children A and B were missing field trip permission slip. 2) Child C was missing the father's employer address, and insurance policy number. 3) Child D was missing first day of attendance, insurance policy number, and field trip permission. Plan of Correction: Files will be updated by tomorrow.
Standard #: 22VAC40-111-690-D Description: Based on observation, the diapering surface was not nonabsorbent. Evidence: The diapering surface was ripped in two places and the absorbent cushion was exposed. Plan of Correction: I will buy a new one this weekend.
