Comments:
A complaint investigation was initiated on 04/24/2018 with on site inspections conducted on 05/08/2018 and 06/20/2018 in response to a complaint regarding supervision & care of children, diapering procedures and nutrition. Based on the information gathered during the investigation the allegations regarding supervision & care of children could not be substantiated. The allegation regarding nutrition was found to be valid. Additional violations were found in the areas of administration and physical environment and equipment. The violations are listed on the violation notice and were reviewed with the provider during the exit interview on 05/08/2018.

Description: Based on record review and interview, the provider failed to ensure that meals and snacks provided by the family day home shall meet USDA guidelines: Evidence: 1. The posted menu did not list a second food component for morning snack on 05/07/2018 and 05/08/2018. a. The snack, for both days, is listed as fruit and water. Water is not a food component. b. Two separate food components are required for snack. 2. The posted menu did not list a second food component for afternoon snack on 05/07/2018. a. The afternoon snack is listed as gold fish crackers and water. Water is not a food component. 3. The provider confirmed the accuracy of the posted menu and confirmed these were the foods served to children.

Description: Based on observation, the provider failed to ensure that children under the age of four years are not served foods that are a choking risk. Evidence: 1. The provider was observed providing a snack of gummy fruit snacks to child 3, age 20 months, at 11:20am on 05/08/2018. a. Gummy fruit snacks are a choke hazard for children as they are sticky, slippery, round and small.

Plan of Correction: No longer giving fruit snacks to all children.

Description: Based on observation, the provider failed to ensure that children shall not be allowed to eat or drink while walking, running, playing or lying down. Evidence: 1. Child 1, age 18 months, was observed napping in a crib with a sippy cup on 05/08/2018. 2. Child 2, age 8 months, was observed napping in a crib with a bottle on 05/08/2018. 3. Child 1, age 18 months, was observed walking around the front child care room with a bottle of milk on 05/08/2018 off and on for approximately 20 minutes beginning upon arrival of the inspector at 11:00am.

Plan of Correction: No child will be allowed to walk around while drinking from a bottle or sippy cup. All kids will be in a high chair or booster seat. No bottles served in cribs.

Description: Based on observation, the provider failed to ensure that bottles are labeled with the child's full name and the date, if more than one infant is in care. Evidence: 1. Child 1 was observed with a bottle that lacked a date and name label on 05/08/2018. 2. Child 2 was observed with a bottle that lacked a date and name label on 05/08/2018. 3. Child 3 was observed with a bottle that lacked a date and name label on 05/08/2018. 4. Child 4 was observed with a bottle that lacked a date and name label on 05/08/2018.

Description: Based on observation, the provider failed to ensure that children under two years of age shall not use pillows. Evidence: 1. Child 1, age 18 months, was observed napping in a crib with a pillow on 05/08/2018. a. Child 1 was napping in a bedroom with the door closed.

Plan of Correction: Remove all pillows under 2 years of age. Leave doors cracked open.
