Technical Assistance:
All background checks are good for 5 years from the date on each form. Please call OBI (office background investigation) 804-726-7884 to obtain your Filed print code. Follow the directions in the email to schedule your appointment for fingerprinting.

Comments:
An unannounced renewal inspection was conducted today between the hours of 9:00 am through 10:30. There were a total of 8 children (24 points) with 2 caregivers. A total of 3 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: toy play, colors and alphabet, bathroom breaks and hand washing. Standard #: 22VAC40-111-120-A
Description: Based on observation and provider interview, the Provider has not maintained a record for each caregiver. Evidence: There is no record for caregiver #1. The Provider stated that her assistant quit, she called in a family member yesterday to assist with care this week.

Plan of Correction: I will be hiring a new assistant, I will get their required documents.

Standard #: 22VAC40-191-60-B
Description: Based on record review and Provider interview, a caregiver was employed before completion of a sworn statement. Evidence: Caregiver #1 was assisting the Provider today, there was no documentation of a sworn statement. The Provider stated that her assistant quit, she called in a family member to assist with care. The caregiver started yesterday and will help this week.

Plan of Correction: I will be hiring a new assistant, I will get their required documents.

Standard #: 63.2-1720.1-B-2
Description: Based on record review and Provider interview, a caregiver was employed before completion of fingerprinting. Evidence: Caregiver #1 was assisting the Provider today, there was no documentation of fingerprinting results. The Provider stated that her assistant quit, she called in a family member to assist with care. The caregiver started yesterday and will help this week.

Plan of Correction: I will be hiring a new assistant, I will get their required documents.
