Technical Assistance:
Standard 22 VAC 40-111-820 (shelter-in-place procedures) and background check procedures, to include out-of-state central registry search requirements, were discussed with the provider during the inspection.

Comments:
An unannounced monitoring inspection was conducted from 1:50pm until 4:00pm. At the time of the tour, there were 6 children in care for a total of 15 points with the provider and one assistant. A sample of 6 children's records and 2 provider/assistant records were reviewed. Children were observed resting quietly during nap time, participating in story time with the assistant and engaging in free play. Infant bottle feeding was also observed. First aid and emergency supplies, documentation of emergency practice drills and family day home postings were reviewed. Medication is not currently being administered and there have been no reported injuries to children in care since the last inspection. Violations in the areas of administration, caregiver training, physical environment and equipment and nutrition are identified on the violation notice and were discussed with the provider during the exit interview.

Violations:
Standard #: 22VAC40-111-920
Description: Based on observation and interview, the provider did not ensure that when meals or snacks are provided by the family day home, the menu for the current one-week period shall be posted in an area accessible to parents. Evidence: 1. The menu for the current week was not posted in an area accessible to parents. 2. The provider confirmed that the menu for the current week was not posted in the family day home.

Plan of Correction: The provider will ensure the menu is posted tomorrow in the family day home.

Standard #: 22VAC40-111-100-B
Description: Based on record review and interview, the provider did not ensure that a child's physical examination shall have been conducted within twelve months prior to attendance for children two years of age through five years of age. Evidence: 1. Child #1 (age 3 years) has a date of enrollment of 6/18/18. The record for child #1 contains documentation of a physical examination dated 5/7/17, which is more than 12 months prior to attendance. 2. The provider confirmed that child #1's physical examination was completed more than 12 months prior to attendance at the family day home.

Plan of Correction: The provider notified the parent during the inspection that an updated physical is needed. Once documentation of the physical is obtained, it will be placed in the child's record.

Standard #: 22VAC40-111-230-A
Description: Based on record review and interview, the provider did not ensure that there is written documentation of each caregiver's orientation training. Evidence: 1. The record for caregiver #1 does not contain documentation of orientation training. 2. The provider confirmed that caregiver #1 was provided with orientation training to the family day home but the training was not documented in caregiver #1's record.

Plan of Correction: The provider will place documentation of orientation training in caregiver #1's record.

Standard #: 22VAC40-111-280
Description: Based on observation and interview, the provider did not ensure that potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents and disinfectants, shall be stored in areas inaccessible to children. Evidence: 1. There was a container of Clorox disinfecting wipes sitting on the changing table in one of the rooms used for child care and was within reach and accessible to the children in care. Clorox disinfecting wipes have the following warnings: "keep out of reach of children", "caution" and "hazardous to humans and animals." 2. The provider confirmed the Clorox disinfecting wipes were located on the changing table and were accessible to the children in care.

Plan of Correction: The Clorox disinfecting wipes were taken off of the changing table during the inspection and placed in an area inaccessible to the children in care.
