An unannounced monitoring inspection was conducted beginning 10:40 and ended at 12:15. There were 9 daycare children in care, ages 6 months - 5. There were two caregivers present. Children were observed playing with age appropriate toys and getting ready for lunch of grilled cheese , carrots and applesauce. Standard #: 22VAC40-111-90-A Description: Based on review of children's records and an interview with provider it was determined that documentation of immunization was not obtained before attendance for all children. Evidence: Child A's start date was 4/30/2018 and the provider obtained the child's immunizations records on 5/18/2018. Plan of Correction: The provider will assure that all parents will have immunization records by the time they enroll.
