Comments:
An unannounced monitoring inspection was conducted on 8/2/18 from 2:05m - 3:15pm. At the time of the inspection there were six children in care, ages two years old to five years old (12 points), with one caregiver present. Records were reviewed for five children and five caregivers/household members. Children were observed during rest period. Medications, emergency plan and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice and were discussed with the provider during the exit interview.

Violations:
Standard #: 22VAC40-111-140-B-3
Description: Based on a review of five caregiver/household member records, it was determined that the provider did not ensure that there is a caregiver present at all times who has a current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department. Evidence: 1. The record for caregiver #1, present and working during the inspection, contained documentation that showed her CPR certification expired in May 2018. 2. Caregiver #1 reviewed her record and confirmed her CPR certification had expired.

Plan of Correction: The provider responded: I will complete a new CPR certification course within the next 30 days. All caregivers will complete a renewal course to update their CPR certification prior to their expiration date.

Standard #: 22VAC40-111-140-B-4
Description: Based on a review of five caregiver/household member records, it was determined that the provider did not ensure that there is a caregiver present at all times who has a current certification in first aid from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current first aid certification issued within the past three years by a community college, a hospital, a rescue squad, or a fire department. Evidence: 1. The record for caregiver #1, present and working during the inspection, contained documentation that showed her first aid certification expired in May 2018. 2. Caregiver #1 reviewed her record and confirmed her first aid certification had expired.
