Comments:
A Subsidy Health and Safety inspection was conducted on 08/10/2018 to review supplemental health and safety requirements. An announced initial inspection was conducted today from approximately 10am-12:40pm. There were 3 children in care (7 points) directly supervised by 1 caregiver with the assistance of the daughter of the provider. A sample of 3 children?s records, 1 caregiver's record and 3 household member's records were reviewed. The children were observed watching television, playing electronics and engaged with the daughter of the provider. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on record review, the vendor did not maintain a written hard copy record of daily attendance that documented the arrival and departure of each child in care as it occurs. Evidence: The departure of each child was not recorded for the month of August.

Plan of Correction: I will use the model form and begin to record the departure of each child today.

Standard #: VENDHOM-000-029-B-3
Description: Based on record review, the evacuation and relocation procedure was incomplete. Evidence: The evacuation and relocation procedures were missing the reunification plan.

Plan of Correction: I will update the add the reunification plan to my emergency procedure plan.

Description: Based on record review, the shelter in place procedure was incomplete. Evidence: The shelter in place procedures were missing the reunification plan.
