Technical Assistance:
Reviewed the renewal application with the provider. Also reminded the provider that a household member that will be turning 18 next year will require fingerprinting and a TB screening.

Comments:
Today an unannounced Renewal inspection took place between 10:40am and 1:00pm. There were 4 children present with the provider and 1 assistant for a total of 10 points within the supervision guidelines. The children were observed playing on the swings and running around in the backyard. When the children came inside they were observed eating lunch, playing freely, pottying and having diaper changes. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information. Evidence: Child #1 file did not contain addresses for their emergency contacts.

Plan of Correction: I'll ask the parents for that this afternoon.

Standard #: 22VAC40-111-200-A
Description: Based on record review, the provider has not orientated a caregiver. Evidence: Caregiver #2 file did not contain documentation of orientation.

Plan of Correction: I'll do it.

Standard #: 22VAC40-111-240-A
Description: Based on observation, areas of the family day home were not maintained in a safe condition. Evidence: 1. The door handle on the pink playhouse in the backyard is broken with jagged edges. 2. There are two metal adult chairs on the playground that are covered in rust.

Plan of Correction: We can take the door handle off the playhouse. We'll get rid of the chairs because we don't use them.

Standard #: 22VAC40-111-680-A
Description: Based on observation, caregivers did not wash their hands after diapering. Evidence: Caregiver #2 was observed changing each child's diaper and did not wash their hands after each diaper change.

Plan of Correction: I told him that too.

Standard #: 22VAC40-111-680-B
Description: Based on observation, caregivers did not ensure that children's hands were washed. Evidence: Child #2 and Child #3 were observed having their diapers changed. Both children were removed from the changing table station and placed directly into their feeding chairs. The children did not have their hands washed after their diaper change or before eating. Child #1 was observed having their diaper changed and their hands were not washed after they were changed.

Plan of Correction: We usually do.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diaper changing surface was not cleaned and sanitized after each use. Evidence: Child #1, Child #2 and Child #3 were observed having their diapers changed. The diaper changing pad was not cleaned or sanitized after each use.

Plan of Correction: We will sanitize it.
