An unannounced Monitoring inspection was conducted in the Family Day Home on August 8, 2018. The Provider reported 17 children are currently enrolled, the children attend on an alternating schedule. There were 7 children in care of the Provider and the Assistant. Based on the ages of the children in care, the points totaled 19. The child care areas were clean, organized and supplied with a variety of age appropriate materials and toys. The children were observed during nap time, free play and afternoon snack. Several children arrived to the fdh after school and several parents were observed waiting on the school bus in order to pick up their child from school; this was the first day of public school. Dismissal procedures were also observed during the late afternoon. A sample of five children's and one caregiver's records was reviewed. A prescribed medication was observed for a child in the fdh. Both caregivers present during the inspection have completed medication administration training (MAT). Documentation of the practiced emergency drills was reviewed. Documentation of a written injury report was reviewed for a child, the Provider had reported the incident to this Licensing Inspector and had completed the on-line report and submitted to the Richmond Licensing Office. Standard #: 22VAC40-111-790 Description: Based on direct observations and interview with the Provider, the Provider failed to post in a visible area and close to the telephone the contact information for the responsible person for emergency back-up care. Evidence: The name and phone number of a designated back-up person in the event of an emergency was not posted in the family day home near the phone. Plan of Correction: The Provider posted the name and phone number of the designated emergency back-up person with the other emergency numbers at the time of the inspection.
