Thank you for your assistance during today's unannounced monitoring inspection. This on-site visit was conducted on August 15, 2018 from 11:30 a.m. through 1:00 p.m. There were four children in the care of the provider for a total of ten points. Four children's records were reviewed and updated. The record for the provider and two household members were reviewed and updated. No medication is being administered at this time. Areas of non-compliance may be found on the violation notice. Areas observed were free choice, hand washing/bathroom and diapering procedures, nap, lunch preparation and lunch. Interactions between the children and provider were observed as positive and the children seem comfortable with the daily routine and schedule. Description: Based on record review the provider failed to have the physician's contact information and a second emergency contact for one child. Evidence: 1. Four children's records were reviewed. 2. Child #1 did not have documentation of the physician's name, address and telephone number. 3. Child #1 did not have documentation of a second emergency contact's name, address and telephone number. 4. The provider confirmed this documentation was missing.

Plan of Correction: I will contact the parent to provide the second emergency contact address as well as the physician's address and telephone number.
