Upcoming weekend training in the Sterling area provided by Infant and Toddler in October discussed.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 11 children were present (29pts) with 2 assistants present when the inspector arrived. Provider arrived shortly after. Children were observed finishing lunch and transitioning to hand washing and preparation for afternoon nap. Observation of the crib room, diapering area and program area was made. Ample supplies, books and toys available for the children and within reach of them to encourage independence in selection. A review of 5 children's records, and 2 staff records was made. Household records will be reviewed at the next inspection along with annual staff training. No medications at this time and no food allergies. This inspection began at 11:15am and ended at 1:30pm. Standard #: 22VAC40-111-900-B
Description: Based on observation, one child was observed laying in a crib with a baby bottle. Evidence: Child #5 was placed in a crib drinking a baby bottle.

Plan of Correction: Staff immediately removed this child from the crib and had them sit at the table to finish drinking.

Standard #: 22VAC40-111-960-E
Description: Based on observation of the physical plant, not all bottles were labeled with the child's full name and date. Evidence: One child's bottle was found on the shelf in the crib room not labeled with the child's full name and date.

Plan of Correction: This bottle was dropped off later by a grandparent. I will ensure that all bottles are labeled with both name and date of each child.

Standard #: 22VAC40-111-280
Description: Based on observation of the physical plant, hazardous substances were found accessible to children in care. Evidence: A lyson cleaning agent was found under the bathroom sink in the bathroom accessible to children.

Plan of Correction: We will remove this cleaning agent immediately and place a locking device on this cabinet.

Standard #: 22VAC40-111-690-D
Description: Based on observation of the diaper changing area, the diaper surface was no longer nonabsorbent. Evidence: The green diaper mat located on the diaper changing table was cracked and determined no longer nonabsorbent.

Plan of Correction: We will remove and dispose of this mat immediately.
