Comments:
The licensing inspector conducted an unannounced renewal inspection on 08/27/2018 from 11:21am to 1:56pm. The children were playing with toys and napping upon the inspector?s arrival, and they were later observed playing with playdough, singing and counting. Today, there were two children, totaling 4 points, present with one caregiver. The caregiver was observed interacting with the children. Interviews were held with the caregiver throughout the inspection. Additionally, when appropriate the inspector interacted with children. Lunch consisted of hamburgers, baked beans, strawberries and milk. The provider does not administer medication. The provider does not provide transportation. The provider does not have a pet. Three children?s records, and two caregivers? records were reviewed during the inspection. Last emergency drill: 07/31/2018 Last shelter-in-place drill: 04/30/2018 The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection. The Department has not yet received a complete renewal application and annual fee from the provider. A complete renewal application and fee must be received by the Department prior to expiration of the license on October 22, 2018 or the license cannot be renewed. Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider did not ensure that two of three children's records contained confirmation of up-to-date information. Evidence: Child #2 was enrolled on 09/20/2016 and the record was last updated 9/16/2016. Child #3 was enrolled on 01/6/2016 and the record was last updated 1/6/2016.

Plan of Correction: As soon as the parents come to pick up children we will review and update information.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider did not ensure that protective covers were installed on all unused electrical outlets accessible to children under five years of age. Evidence: 1. There was an uncovered outlet located on the lower part of a wall next to a shelf in the activity room. 2. There was an uncovered outlet located above the sink in the bathroom.

Plan of Correction: Corrected during the inspection.
