Comments:
Licensing inspector arrived at 11:23am and departed at 2:05pm to conduct a licensing inspection and a Subsidy Health and Safety inspection. At the time of the tour there was thirteen children in care with 2 staff (29 points)Children were observed napping, freely playing and school age children reading The sample size consisted of 6 children records and 2 provider record. Areas of non-compliance was reviewed with the provider.

The inspection was amended on 12/07/2018.

Violations:
Standard #: 22VAC40-111-900-B
Description: Based on observation, the licensee did not ensure children were walking or lying down and playing while eating or drinking.

Evidence:
During the inspection conducted on August 21, 2018, there were three children in cribs drinking milk while standing up , walking around and playing for approximately 20 minutes. Provider stated they were in the cribs to take a nap.

Plan of Correction: All children are drinking then milk in a seat

Standard #: 22VAC40-111-100-A
Description: Based on review, provider did not ensure 1 of 6 children's records had a copy of a physical examination within 30 days of child beginning attendance.

Evidence:
During the inspection conducted on August 21, 2018, the record for child #1 ( start date 07/16/2018) did not have documentation of a physical examination.

Plan of Correction: Parent provided physical.

Standard #: 22VAC40-111-60-B
Description: Based on 2 of 6 records reviewed , provider did not ensure children's records has documentation of required information.

Evidence: During the inspection on August 21, 2018, the following records did not have required information:
Child #1 did not have medical insurance policy number.
Child #4 did not have medical insurance policy number .
Child #5 did not have second emergency contact with address, city , state and zip code.
Child #6 did not have two emergency contacts.

Plan of Correction: Parents provided the information.

Standard #: 22VAC40-111-240-A
Description: Based on observation , the licensee did not ensure areas and furnishings of the family day home, inside , shall be maintained in a clean, safe, and operable condition.

Evidence: During the inspection conducted on August 21, 2018, the following were observed in the house:

The coat rack used for children belongings was detaching from the wall. The screws were visible .

There was white peeling paint on the railings of the three white cribs.

Plan of Correction: Hook was replaced on the wall. Railing where cover with railing. Cover protector.

Standard #: 22VAC40-111-280
Description: Based on observation, the licensee did not ensure poisonous substances are inaccessible to children.

Evidence:
During the inspection conducted on August 21, 2018, there was a can of disinfectant on the couch.

Plan of Correction: All poisonous substances are keep in a lock cabinet Disinfectant was put in a lock cabin . Bag of grapes where throw away.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the licensee did not ensure child-resistant larger than 1-1/4 inches in diameter be installed on all unused electrical outlets.

Evidence:
During the inspection conducted on August 21, 2018, there was an electrical outlet without a protective covers.

Plan of Correction: All outlets where cover.

Standard #: 22VAC40-111-510-C
Description: Based on observation, the licensee did not ensure the cribs and mats were at least 12 inches from each other. Evidence: During the inspection conducted on August 21, 2018, two cribs were side by when occupied and then cribs were moved when children were napping in crib the cribs were approximately 10 inches. Upon arrival a occupied cot with two children laying on it was next to the occupied crib. There was not at least 12 inches space. The two cots in living room were not spaced 12 inches when occupied.

Plan of Correction: All cribs and cots are been we inches apart

Standard #: 22VAC40-80-120-A-4
Description: Based on observation and record review, the licensee did not operate within terms of license.

Evidence:
During the inspection conducted August 21, 2018, the center license capacity was for 12 children there were 13 children present.

Plan of Correction: One of the children was withdraw
