Comments:
An unannounced monitoring inspection was conducted today from 10:37am-1:00pm. There were 6 children (17 points) supervised by 2 direct care staff. The home, 3 caregiver records, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing with cars, Legos, on an indoor slide, and reading books. Handwashing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-111-750-B
Description: Based on observation, diaper creams were not kept in a location inaccessible to children. Evidence: An organizing tray containing diaper creams was attached to the side of the diaper changing table, making them accessible to children during diaper changes.

Plan of Correction: I will put them away today.

Standard #: 22VAC40-111-790
Description: Based on observation, required telephone numbers were not posted. Evidence: A 911 or local dial number for police, fire, and emergency medical responders was not posted.

Plan of Correction: I will get it today and post it.

Standard #: 22VAC40-111-830-A
Description: Based on record review and caregiver interview, the emergency evacuation procedures were not practiced monthly. Evidence: There was no documentation of fire drills completed during the months of January, March, May, and July of 2018.

Plan of Correction: I will do the fire drills within the first week of every month.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 2 of 3 children's record did not contain all of the required information. Evidence: 1. Child #1's (start date: 5/7/2018) record did not contain documentation of a second emergency contact person, and proof of identification. 2. Child #2's (start date: 11/7/2016) record did not contain documentation of a physical completed by a physician.

Plan of Correction: I will get the missing information.

Standard #: 22VAC40-111-680-B
Description: Based on observation, caregivers did not ensure that children's hands were washed with liquid soap and warm running water after toileting or diapering. Evidence: 1.Caregiver B (date of hire: 2.13.2017) did not wash Child #2's (dob: 6/17/2016) hands after changing the child's diaper. 2. Caregiver B stated that the child's hands were cleaned with wipes after performing the diaper change. However, warm running water and soap were accessible.

Plan of Correction: We started today.
