Comments:
The following inspection took place between the hours of 10 am through 12 pm. Today, I met with the Provider at the FDH located in Chantilly, Virginia. This unannounced Non-mandated Monitoring Inspection (NMI) serves as a follow-up to the initial inspection conducted previously by the Division of Licensing. This visit was needed to review compliance of any area left undetermined or any standard that was cited during the previous inspection. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on review, 1 of 5 children's files reviewed indicated that not all of the required information / documentation was contained within each child's record. Evidence - 1. On the date of inspection (08/30/2018), 1 of 2 emergency contacts were listed for Child #5. 2. While complete information for the first emergency contact for Child #5 was listed, the space for the second was left blank.

Plan of Correction: The Parent of Child #5 shall be contacted and the information for the second emergency contact shall be placed on file.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on review and interview, 1 of 5 household member's files reviewed indicated that a criminal history record check was not obtained for a person living in the home who has now become 18 years of age. Evidence - 1. On the date of inspection (08/30/2018), based on the information on file, a fingerprint check had not been conducted on household member #2. 2. The Provider stated (during an interview conducted on the date of inspection) that household member #2 had recently turned 18 years of age and that the fingerprint check was yet to be obtained.

Plan of Correction: Household member #2 shall schedule an appointment and the results shall be obtained and placed on file.
