Comments:
An unannounced monitoring inspection was conducted on 09/10/2018 from 11:35am - 2:00pm. There were two children in care with the provider. The ages of the children counted for a total of four points. The inspector observed transportation standards. Children were observed playing with toys and games, with the provider, in the child care room. The sample size included one child and two adult/caregiver records. The violations are listed on the violation notice and were reviewed with the provider at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-111-730-D
Description: Based on observation, the provider failed to ensure that medication shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. A pink lunch container / storage bag was observed placed on the parent sign in shelf in the child care room. The bag was half open at the top revealing a container of children's Tylenol inside of the bag.

Plan of Correction: Medication will be returned today. The parent will be asked to not leave any medication in children's bags. The bag with the medication was put away.

Standard #: 22VAC40-111-800-B
Description: Based on review, the provider failed to review the emergency plan at least annually and update the plan as needed. The provider shall document in writing each review and update to the emergency plan. Evidence: The most recent review of the emergency plan for the FDH was last done on 08/08/2017 according to written documentation on file.

Plan of Correction: The emergency plan will be reviewed, and updated if needed, today.

Standard #: 22VAC40-111-830-A
Description: Based on record review, the provider failed to ensure that the emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care. Evidence: 1. The written emergency log for the family day home indicated that the most recent fire drill for the family day home was conducted on 05/16/2018. a. Attendance sheets for the family day home indicate that children were in care during the months of June, July and August of 2018.

Plan of Correction: A fire drill will be done in September and each month going forward. A fire drill will be done this afternoon.

Standard #: 22VAC40-111-1010
Description: Based on observation and interview, the provider failed to ensure that during transportation of children each child is in an individual car seat or individual and appropriate restraint in accordance with Virginia law. 1. The inspector observed the provider arrive to the family day home at 11:35am with two preschool children in her vehicle. a. The inspector observed one car seat in the vehicle. 2. The provider confirmed that there was only one car seat in the vehicle and that one child, child 1, was not in the age appropriate child restraint device during transportation. a. Child 1 is two-years of age.

Plan of Correction: Transportation will not longer be provided unless a car seat is available.

Standard #: 22VAC40-111-990-B
Description: Based on interview, the provider failed to ensure that during transportation of children the driver has the following in the vehicle: *A copy of the parent's written permission to transport the child; *A copy of each child's emergency contact information; *Emergency supplies (first aid kit). Evidence: 1. The provider arrived at the family day home at 11:35am with two preschool children in her vehicle. a. The provider stated she had been up to the store. b. The inspector asked to review the following: first aid kit, a copy of the parent's written permission to transport the child and a copy of each child's emergency contact information. 2. The provider stated that she did not have a first aid kit in the vehicle nor did she have the children's emergency contact information or written parent authorization for transportation.

Plan of Correction: Transportation will no longer be provided.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider failed to obtain for for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening . Evidence: 1. The most recent TB screening for the provider and household member #1 was conducted on 04/18/2016 according to written documentation on file. a. The provider confirmed that a more recent TB screening was not on file.

Plan of Correction: The provider stated: The TB screening has been obtained however a copy can not be located. The provider stated she will obtained a copy of the most current screening today or tomorrow.
