The outdoor play area could not be inspected today due to the current heavy rain.
An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 4 children, totaling 10 points. Children's records, background checks, health records, emergency preparedness items, children's nutrition, programming, and the physical plant were inspected today. It was reported to the Inspector that no children's medications were on site today. The children were observed playing with toys, dancing, and singing to music. The children were to be served chicken nuggets, french fries, applesauce, and milk for lunch later today. This inspection was completed between approximately 9:45am and 11:30am. Standard #: 22VAC40-111-830-C Description: Based on a review of emergency preparedness items, it was determined that the provider has not accurately documented all required practice drills. Evidence: The provider did not document each emergency evacuation drill during the 2018 calendar year. Plan of Correction: I documented every tornado drill, but didn't document the fire drills correctly. I will document every drill correctly. We did practice a fire drill every month.
Standard #: 22VAC40-111-920 Description: Based on observations, it was determined that the provider's menu was not completed as required. Evidence: Upon arrival, the menu for the current one week period was not posted. The provider did not update the menu for today. The menu indicates that the children were to be served brussel sprouts, but the provider is serving applesauce today. The provider did not update this change on the menu. Plan of Correction: The menu was posted and updated during the inspection.
Standard #: 22VAC40-111-50-A Description: Based on a review of administrative documents, it was determined that the provider has not completed all required documentation. Evidence: Upon arrival, the documentation of daily children's attendance was not completed. Plan of Correction: Today's attendance was immediately completed during the inspection.
Standard #: 22VAC40-111-180-A Description: Based on a review of adult health records, it was determined that the provider does not have documentation of all required TB Health Screenings. Evidence: There is no documentation of a TB Health Screening for adult household member A. Plan of Correction: I have lost his TB report. I will make sure he gets another TB test done.
Standard #: 22VAC40-111-340-C Description: Based on observations, it was determined that the children's bathroom was not equipped as required. Evidence: Upon arrival, the children's bathroom was not equipped with toilet paper. Plan of Correction: The toilet paper was resupplied during the inspection.
Standard #: 22VAC40-111-530-B Description: Based on observations, it was determined that one crib mattress is not covered with a tight fitting bottom cover. Evidence: One crib mattress has a very loose sheet. The sheet gaps and gathers and hangs out of the sides of the crib because it is too large. This sheet is too large to fit the mattress properly and fit tightly. Plan of Correction: The sheet was changed during the inspection.
