An unannounced monitoring inspection was conducted beginning at 9:30 and ending at 10:45. There were 7 children in care including two resident children. Children were observed working at table tasks, Montessori activities, at circle time and having snack. Reviewed training hours - exceeds Licensing regulations Reviewed Emergency drills and shelter in place - up to date Reviewe staff and family files Reviewed 5 children's records - see violation notice. Description: Based on a review of 5 children's records, it was determined that not all records that were reviewed were complete. Evidence. Child #1 is missing Child Record form pages 1 and 2, physical and shot record. Child #2 is missing insurance policy Number; Child #3 is missing Information for parent page 1 & 2, second emergency contact and phone number for 1 emergency contact,

Plan of Correction: All children's records will be complete by the end of the month.

Description: Based on review of family and staff files it was determined that several TB tests had expired. Evidence: Provider and family Member A have TB yests that expired in August, Staff 1 has a TB test that expired in July.

Plan of Correction: All will get Tb tests by the end of the month.
