54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
Consultation was given on obtaining an additional 6 hours of training for both assistants prior to October to meet the 16.0 hours of required training per year as well as promoting music and language with very young children, walking while eating snack and MAT certification.

Comments:
An unannounced renewal study was conducted today from 10:00-11:00. Nine children were present with three caregivers for twenty none points which is within points for supervision. Dedicated space in the lower level of the home has two rooms for care, a bathroom and small foyer for drop-off. Reviewed background check requirements for all caregivers and household members, staff training, children records and emergency preparation. Standard #: 22VAC40-111-870-G
Description: Based on observation, the eating surface is not able to be sanitized as the children were eating snack on the floor. Evidence: Snack was served to older children in cups that children could eat from but they were eating while sitting on the floor rather than at a table.

Plan of Correction: A table was be placed out and used for all snacks and meals and be sanitized prior to eating.

Standard #: 22VAC40-111-60-A
Description: Based on record review, 1 of 3 children's records are incomplete. Evidence: Child #2 is missing documentation for 2 parts of the agreement form (allergy information and information for special needs)

Plan of Correction: The parent will complete the information or mark N/A.

Standard #: 22VAC40-111-90-C
Description: Based on record review, 1 of 3 children's records is missing updated immunization record. Evidence: Children under the age of two years of age should update their immunization record every 6 months. Child #1 does not have an updated record since 3/2018.

Plan of Correction: The provider will request an updated immunization record from the parent.
