Comments:
An unannounced Renewal inspection was conducted beginning at 9:15 and ending at 11:30. There were 6 children in care including two who are resident children. Children were observed playing with toys both upstairs and in the downstairs daycare room. Reviewed 5 children's files - see Violation notice Reviewed 5 adult files - complete Emergency drills were posted Menu is posted. There are no children on medication. provider is not MAT trained and cannot give medication. Standard #: 22VAC40-111-50-A
Description: Based on observation and interview with the provider, it was determined that there was no record of attendance Evidence: The provider could not locate the attendance book. .

Plan of Correction: The provider will keep the attendance book in one location.

Standard #: 22VAC40-111-60-B
Description: Based on a review of 5 children's files, it was determined that not all files were complete. Evidence: Child #1 does not have insurance policy Number and physical; Child #2 does not medical insurance number and phone number for second emergency contact; Child #3 Does not have insurance policy number, shot record

Plan of Correction: Provider will review all files for complete information.
