An unannounced monitoring inspection was conducted today between the hours of 8:30 am through 10:00. There were a total of 6 children, including Provider's own child (17 points) with 2 caregivers. A total of 5 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: toy play, coloring, blocks and diapering. Description: Based on record review, staff records do not contain all the requirements. Evidence: The assistant's record does not contain 2 references or the person to contact in an emergency. (date of employment 8/2/18)

Plan of Correction: I will have the assistant complete the staff form and obtain 2 references.

Standard #: 22VAC40-111-60-B
Description: Based on record review, each child's record is not complete. Evidence: Child # 4 record does not have documentation of 2 emergency contact addresses. Child #4, date of first attendance 9/10/18 does not have documentation of immunizations.

Plan of Correction: I will get this information from the parents.

Standard #: 22VAC40-111-200-C
Description: Based on record review, there is no documentation that the assistant has been provided the required orientation training. Evidence: There is no documentation that the assistant has received the required orientation. (date of employment 8/2/18)

Plan of Correction: I will have her sign the orientation form.
