54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced renewal inspection was completed on 9/19/2018. There were five children in care with the provider and an assistant supervising at a total of ten points. Five children's records, the provider's record and one household member's (assistant) record were reviewed. The inspector arrived at the provider's home at 8:00 A.M. and departed at 12:30 P.M. The children were observed during free play times, during morning snack, outside time, lunch and start of nap. There was discussion with the provider about the following: using household member as assistant and completing all required paperwork and training, finger prints requirements. Obtaining MAT training.

Standard #: 22VAC40-111-210-A
Description: Based on assistant's record, the licensee failed to ensure that the required 16 hours of annual training was obtained. Evidence: 1. The assistant had four hours of annual training for 2017-2018.

Plan of Correction: The licensee will make sure that the assistant obtains a certificate as each training attended.

Standard #: 22VAC40-111-320-G
Description: Based on review of the most recent chimney inspection, the licensee failed to ensure that an inspection was completed every year. Evidence: 1. The most recent inspection was completed on 11/3/2016.

Plan of Correction: The licensee stated that a call has been made to the local fire department and to a local business. All annually inspections will be completed on schedule.

Standard #: 22VAC40-111-510-A
Description: Based on review of the nap time items, the licensee failed to ensure that all children had a rest mat or cot. Evidence: 1. Observed that three of the five children in care did not have a rest mat. There had sleeping sacks that had padding. The standard required mats, cots or beds for the children in care.

Plan of Correction: The licensee will have the parent or licensee will obtain mats for the three children that do not have one. All children in the future will have mats or cots to rest on.
