54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification dated 09/19/18.

Comments:
An unannounced monitoring inspection was conducted from 10:00am-12:00pm. Upon arrival there were 12 children in care, totaling 33 points with 4 caregivers. All children's records were reviewed, in addition to 3 caregiver records. The dog's record was also reviewed. Children were observed during lunch and nap. The children decorated pictures of cupcakes. The provider had decorated cupcakes for the children to eat with their afternoon snack.

Violations:
Standard #: 22VAC40-111-320-I
Description: Based on observation the licensee did not ensure there were no open and obvious fire hazards. Evidence: There was a 2 wick candle burning in the kitchen on top of the kitchen island where the children's food was being prepared.

Plan of Correction: The following corrective action was provided during the exit interview: The licensee blew the candle out during the inspection. The licensee will not burn candles during the hours of operation of the daycare.

Standard #: 63.2-1720.1-B-2
Description: Based on a record review the licensee did not ensure all requirements when submitting fingerprinting. Evidence: Caregiver #3 (hired on 08/31/18) did not have documentation of a fingerprint background check. Caregiver #3 was observed working with the children during the inspection.

Plan of Correction: The following corrective action was provided during the exit interview: Caregiver #3 has gone to field print once and then went to another location since results did not come back. The licensee will call the Office of Background Check Investigations to follow-up on the results immediately.

Standard #: 63.2-1720.1-B-3
Description: Based on a record review the licensee did not ensure all requirements when submitting fingerprinting. Evidence: Caregiver #2 (hired 08/20/18) listed having lived in three states outside of Virginia in the last 5 years. The licensee did not have documentation that the correct Central Registry Checks for each of these states has been requested.

Plan of Correction: The following corrective action was provided during the exit interview: The licensee has requested the 3 central registry checks, however did not keep copies of the request. The licensee will recontact each state to obtain written confirmation they have been sent and follow-up on results.
