Technical Assistance:
Discussed with Provider alternatives for land line phones, current trainings available.

An unannounced mandated Renewal inspection was conducted today from 10:00am to 1:00pm. There were 4 children (10 points) and 1 caregiver present. Children were observed having snack, engaging in child's choice activities, practicing numbers and letters, and coloring. Evacuation drills, first aid and emergency supplies, 4 family pet rabies were all current. There were ample equipment and supplies available for children. Description: Based on observation and interview with provider, menus for meals provided by family day care home are not being documented weekly. Evidence: Interview with the provider revealed she does not post weekly menus for parents.

Plan of Correction: I'll start doing them

Description: Based on child's record review, observed the provider did not have up-to- date record for an enrolled child. Evidence: Child#1's record was not signed and dated at time of enrollment.

Description: Based on record review, children's records did not contain documentation of parent receipt for the following items: Evidence: 1. Child#1 record did not have documentation of the provision of emergency preparedness and response plan indicating the relocation site for emergencies.

Description: Based on review of caregiver's record and interview, caregiver did not obtain 16 hours of annual training. Evidence: Provider was missing 4 hours of annual training for 2017 year.

Description: Based on direct observation, home is not free of rodent infestation. Evidence: A rat was observed in the backyard play area, roaming on the landscaping barriers, and under a child's toy car.

Description: Based on observation, caregiver did not wash their hands with soap and water after diapering. Evidence: Caregiver changed Child#1's diaper, and did not wash their hands.
