Comments:
An unannounced monitoring visit was conducted today between the hours of 9:45am through 11:45am with the provider on site. There were 4 children (11 points) with the provider and assistant. A sample of 5 children's files and 2 staff/household member's files were reviewed. The children were playing with toys. Positive interaction was observed between the caregivers and children. The home is clean and organized. There were no medications on site today. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-111-830-A
Description: Based on review and interview, the emergency evacuation procedures were not practiced monthly with all caregivers and children. Evidence: 1. The provider had documentation of monthly fire drills for every month this year, but did not have record of doing a fire drill in August 2018. The provider stated he did not know a fire drill had to be conducted that month if another type of drill was practiced.

Plan of Correction: I will do a fire drill each month.

Standard #: 22VAC40-111-50-A
Description: Based on observation, the family day home did not keep a complete written record of children in attendance each day. Evidence: 1. The provider had a written record of attendance, but did not have child #2 (arrived at 7:30am) and child #3 (arrived at 8:45am) on the attendance sheet during the inspection that began at 9:45am.

Plan of Correction: I will make sure all children attending are on the attendance sheet.

Standard #: 22VAC40-111-70-B
Description: Based on record review, the provider did not obtain the parent's written acknowledgment of the receipt of the information regarding the policy for the administration of medications. Evidence: 1. Child A had a copy of the policy for the administration of medications, but did not have the parent's signature and date on the form.

Plan of Correction: I will have the parent review and sign the form.
